Four contract workers were hurt in a gas release during welding on a unit that was down for maintenance, six days after the Oct 9 blast that killed two; Motor Oil paused preventive maintenance until Oct 19. Two events in one turnaround point straight at hot-work permitting, gas testing and contractor control, which is where underwriters should push before the next turnaround is bound.
